refactor(config): rename version fields in upgrade config

Configuration changes:

• upgrade.version → upgrade.format (output format: tag/hash/major)
• upgrade.actions.<name>.version → upgrade.actions.<name>.constraint (semver constraint: ^1.0.0, ~2.0.0)
