Single Sign-Up Form

Right now when someone wants to join hack night they have three forms to fill out:
- RSVP on Zoom
- Join Slack
- Sign-up For Weekly E-Mail Newsletter

Ideally I'd like to have a single sign-up form that folks can fill out that does all three of these things in one shot.

I would also like to replicate this for Civic Tech Coffee.
